2009 WAD Data Entry Form and Instructions

To simplify the uniform reporting of testing results at the conclusion of the Testing Millions Campaign, we are providing all testing partners with a one-page Testing Data Form. The form can either be emailed or faxed to the contact information indicated at the bottom of the form. The Testing Data Form and Instruction Guide can be downloaded at the following links:

We need to receive your testing results no later than December 11, 2009 to include them in the worldwide totals, which we will be reporting on the Campaign website and publishing in the press after World AIDS Day.
